If you or your family are experiencing homelessness and you have not yet completed a Smart Path Assessment (also known as a VI-SPDAT), please visit one of the Access Points listed below. You can also download a list in English here or in Spanish here (last updated 5/8/2018).
If you have already completed a Smart Path Assessment and need to update your contact information, or if you meet any of the following conditions, please contact the Smart Path Referral Specialist Habiba Rotter at (831) 226-2518.
- If you have a life-changing event, such as
- Having minor children join the household.
- Being diagnosed with a chronic illness or disability.
- You are no longer interested in being considered for housing.

Adult Assessments are provided Mondays through Fridays, 1 PM to 4 PM. Call, text, or email Brian Lands, Assessment Specialist, at (831) 350-1106 or firstname.lastname@example.org for an appointment. Appointments can also be obtained at the Homeless Services Center Safety Kiosk from 8:30 AM to 9:30 AM.
Family Assessments are provided Tuesdays 1 PM to 3 PM at the Rebele Family Shelter. Please bring photo ID for each adult member of the family.
